허악전돌증 환자의 악교정 수술후 시간경과에 따른 안모 연조직 변화에 관한 연구

Abstract
결론
저자는 하악전돌증으로 진단받고 하악지 골절단술을 시행받은 33명의 환자를 대상으로 수
술전과 수술후에 주기적으로 촬영된 측모 두부규격 방사선 사진을 계측하고 연조직 안모의
변화양상을 수술에 의한 경조직의 변화량과 비교, 분석하므로써 다음과 같은 결론을 얻었다.
1. 하악지 골절단술에 의해 하악골이 후방이동될때 수술에 의한 하악골의 위치 변화는 1
년에서 2년 사이와 변화량이 많은 경향을 보였으며 2년 경과시 시상면상에서의 회귀율은 B,
Pg, Me에서 각각 30% ,32% ,29%로 나타났다.
2. 하악지 골절단술 시행후 2년 경과시 해당 경조직 변화량에 대한 연조직의 변화량은 Si,
Pg, Me이 모두 약 90%의 비율을 나타내었다.
3. 수술에 의한 하악골 계측점의 이동량은 하순 및 이부연 조직의 전후적 위치변화에 대
해장기적으로 유의한 연관성을 보였다.
4. 수술에 의한 Pg의 변화량을 기준으로 연조직 Pogonion 변화의 양상을 예측하는 것은
유용하였고 술후 2년 경과시 그 연관성은 감소하였다(r2= 0.46).)
5. 하악지 골절단술을 이용한 하악골 후방이동시 시상면상 상순의 위치는 2년 경과시 술
전에 비해 약간 전방에 위치하는 경향을 보였으나 유의한 변화는 아니었고 수직적으로는 일
정한 변화를 찾을 수 없었다.

This study was performed to evaluate the change of the soft tissue facial profile after
mandibular set back surgery during time intervals. For this study 33 patient, 8 males
and 25 females, were selected and their lateral cephalograms were taken and analyzed
periodically.
Hard and soft tissue changes during postoperative time intervals, correlation between
surgical skeletal changes and postoperative soft tissue changes, and prediction for
long-time soft tissue changes were established through varying statistical methods.
The results were as follow :
1. There were meaningful changes of anteroposterior skeletal position at 6 months and
2 years after mandibular set back by mandibular ramus osteotomy. Two years
postoperatively. there was 30%, 32%, 29% relapse on B point, pogonion, menton each.
3. The movements of the mandibular landmarks in correlation to anteroposterior
position of the lower lip and soft tissue of the chin showed to be effective on a
long-term basis.
4. Using surgical changes of pogonion, prediction of changes in soft and hard tissue
pogonion was useful and the coefficient of determination was 0.046 each and their
reliability decreased 2 years postoperatively.
5. The upper lip position after the mandibular set back surgery was somewhat
anterior 2 year postoperatively, but that has no statistical meanings.
